Yeah its unique to Australians and only came into effect as part of the US/AUS free trade agreement.
I suspect he may have been questioned on some of his living circumstances, which probably left the customs agent thinking he planned to stay in the US indefinitely. Its really important to stress that you are aware you are here temporarily and provide proof that you have roots to Australia during your visa application/renewal, as well as when questioned by customs agents when enterting the US.
US customs agents have always been pretty strict on this, even pre trump. However, I suspect its even more strict now.

I suspect the exact same thing. I was on an E3 for two cycles and my lawyers always instructed me that, if asked, to state that I had no intention to stay here permanently and I was expecting to leave at the end of my visa period.
